Stephen Canfield is not dead. According to Heroes Interactive, his vortices have a "dimensional pull".

+ While being in another dimension, he may be able to create another vortex to come back.
- The hole he created is too small, possibly, in the process of entering, his body is squashed in the hole.
- He may have been killed by the process of entering the other dimension, or something or someone waiting for him on the other side.

+ Maybe waffles were waiting for him in the other side and helped him coming back.

- He may not be able to return to the same dimension. If the other dimension is not in the same time-space continuum, he can be considered dead in terms of relativity if he cannot return before he dies of starvation, thirst or old age. + We exactly don't know what we can find inside a black hole.
